Press the “Submit” button
Within 5 minutes, you will receive an e-mail containing the registration information you just entered
Write a check, payable to “CAFAM Chinese School,” for the total amount shown
Print out the e-mail, initial and sign the included waiver (one for each student), and send it, with your check, to:

The envelope with your complete printed registration and check must be postmarked by September 28, 2011, or a late fee will be applied. No registration will be accepted after October 8, 2011.
Late registration will be accepted — with a $25 penalty — between September 28 and October 8, 2011. The only exceptions will be for families brand-new to the area and to families with a newly adopted child. No walk-ins will be accepted on the first day. We thank you for accommodating this very necessary policy.
Waiver
After you press submit, your e-mail will include the following waiver, which needs to be completed for each student.
Photography
I understand that the Chinese American Friendship Association of Maine (CAFAM), Chinese Dance Performance & Education Program of Maine (CDPE), CAFAM Chinese School or their agents may use photos or videotapes of the above named child(ren)’s participation for promotional or educational purposes.
One of the following must be circled and initialled:
I DO Agree to Photo Permission – Initial [_____]
I DO NOT Agree to Photo Permission – Initial [_____]
Physical activities
I certify that the above named minor child(ren) is (are) in excellent health and may participate in strenuous physical activity. I hereby give permission for the above named child(ren) to participate in Chinese dance activities during Chinese School and any CAFAM or CDPE special programs.
I hereby agree to release, discharge, indemnify and hold CAFAM, CDPE, Chinese School and their representatives, consultants, and all persons acting under their permission or authority harmless from any liability claims, demand costs or damages arising out of said program activities which my minor child(ren) might sustain during his/her/their participation. Permission is hereby granted for my child(ren) to receive emergency medical treatment and be transported to a local hospital, if needed.
I certify that there are no limits to my minor child(ren)’s participation except as stated in writing above. I have read this document carefully, and sign it voluntarily with full knowledge of its significance.
